It is important that you fill out the registration fully and completely.

In particular if you are registering a child and you are the child’s Parent or Guardian you must already be registered with the surgery and you need to complete the section that asks for your date of birth.

Failure to do so will inevitably delay us being able to register your child. Thank you for your attention.
